Guidance for Amendments to the Test and Trace form

• You can change any of the wording from the default form installed - we’d expect you to at least change the guidance notes page / error message presented when they’re ineligible
• The form covers the standard rules set out by the DHSC and not discretionary schemes as the rules that a council wants to adopt could be different from one council to another
• The evidence upload buttons for proof of earnings/bank statements have a check box below them for customers to declare they’ve provided the correct evidence in support of their application (they will still be able to submit it without uploading evidence but cannot submit it without agreeing to that they have provided the necessary evidence and won’t get any money if they haven’t)
• The declaration/consent sections are all editable by you and should reflect your council’s needs
• The content on the page that is shown if a person is eligible should be edited by you to represent your needs/guidance you’d like to provide
